All football players need to submit the following forms before they will be given equipment or allowed to
participate this fall: current physical, medical card, parent consent/proof of residency, proof of insurance.

Due Date #1 is July 11th. Submitting forms by Due Date #1 clears the athlete
of all of the endurance components of tryouts.

| All athletes must submit a yearly fee to Montgomery County Public Schools before being allowed to participate in MCPS sports. Forms are mailed to the home each summer, or you can get it here as soon as it's available. Remember, the fee and form need to be included with your other forms to be eligible to play. Football staff and volunteers will ensure that players' ECA fees are submitted in a timely manner so that ALL players will be eligible. |
